At your first appointment please print and fill out the forms that corresponds to you: New Client or Insurance Client Intake Forms and bring to your appointment. If you don't have a printer, the forms will also be available in the office to fill out. Please arrive 5-10 early to fill out.

Before & After Treatment Care Instructions will be provided during your first appointment. 

If you are coming in for GRS treatments, please fill out and print the Surgical Area Electrolysis form .
